Monthly Logs & Expenses

Per the instructions provided by our Program Director, please log in to Civicore to submit your monthly logs and expense information.

All logs and expense information should be submitted at the beginning of every month for the month prior.

After submittal, you must send your expense receipts to our Program Director by email, fax, mail, or (if legible) text message. 

Expenses can not be reimbursed without both a completed log and receipts.

Please download our forms below to apply to become or get mentor, and send to The Center Foundation per the instructions provided:

• Mentor Application: To apply to become a mentor for WomenCare, YPSN (Young Parents Support Network), or TeenCare, please complete our Mentor Application. 
• WomenCare Partner Application: To apply to get a mentor for Womencare, please complete our Partner Application.
• Teen Program Mentee Application:  To apply to get a mentor for YPSN or TeenCare, please complete our Mentee Application.
